Medical Information
Parents or guardians should immediately inform the school about any significant medical changes or conditions affecting their child by contacting the school office. This includes informing the school about new diagnoses, changes in medication, or any new health care needs that may impact the student's learning or require specific accommodations. If you require your child to take medication during school hours, you may need to complete a Medication Authority Form and a Medical Management Plan (signed by an AHPRA registered medical/health practitioner) or supply a condition specific action plan signed by your doctor.
